About Scar Revision

Scars caused by an accident, burn, or surgery may heal wider or irregularly. During scar revision, the old scar is surgically removed and re-sutured using a refined technique, so that the scar is less noticeable.

  • The costs depend on the size of the scar. A scar correction is available from €450. During a free, no-obligation consultation, a plastic surgeon discusses your wishes and the options, and you receive a transparent cost overview.
  • No, a scar will never disappear completely. The aim of the treatment is to make the scar as inconspicuous as possible.
  • A scar needs about 1 to 1.5 years to settle completely. Good scar care (sun protection and scar cream) promotes good healing.
  • At Boerhaave, a scar correction is performed by experienced plastic surgeons with an eye for safety, quality and a careful result. You can count on personal guidance and comprehensive aftercare, so that you know where you stand throughout the entire treatment process.

Here's how the treatment works

Here you will find all the important information about this treatment, from the consultation and the treatment itself to follow-up care, the results, and potential risks.

  • When you sustain a wound—for example, as a result of an accident, a burn, or surgery—a scar will form during the healing process. It takes at least one year for a scar to mature. With normal healing, a narrow, uncolored scar line usually develops.

Sometimes, however, a scar may become wider than it was originally, or become adhered to deeper structures beneath the skin. In such cases, surgical correction may be considered. It is important to know that a scar will never disappear completely; the goal of treatment is to make the scar as inconspicuous as possible. After surgery, the new scar will also undergo a maturation process lasting at least one year. In the early stages, the scar may actually become more noticeable for a time and, in some cases, widen again. During a no-obligation consultation at the outpatient clinic, the plastic surgeon will assess your scar and discuss the possible treatment options with you.

In some cases, a scar does not mature in the usual way. The scar may then thicken or even grow beyond the original wound edges. We refer to this as a hypertrophic scar or a keloid. In most cases, treatment for these scars is non-surgical and consists of corticosteroid injections, pressure therapy, silicone patches, or, in some cases, radiation therapy.

However, these non-surgical treatments are not offered at the Boerhaave Clinic. We recommend that you contact your primary care physician for a referral to an appropriate specialist.

    How does scarring occur?
The risk of scar formation or excessive scarring can have various causes. For example, certain parts of the body, such as the shoulder, back, and sternum, have an increased risk of scar formation. We also observe that excessive scarring occurs more frequently in people with dark skin tones or very fair, reddish skin tones.

In addition to the location on your body and your skin type, the direction of the scar relative to your skin’s tension lines can also play a role. On the face, this can be clearly seen when the facial muscles are tensed.

The formation of the scar and the tension on the wound edges during healing are other factors that can contribute to scarring. For example, increased tension on the wound as it closes usually results in a wide scar.

  • Plastic Surgeon
Your consultation at Boerhaave will always be with the plastic surgeon who will be treating you.
During this consultation, you can discuss your wishes, photos will be taken for your medical record, and your scar will be examined. Based on this, the most appropriate treatment method for you will be proposed and discussed with you. In front of a mirror, the details of the procedure and what you can expect in terms of the final result will be explained once more.

At the end, your consultation will be summarized in a treatment plan, and the total costs will be discussed with you immediately. This consultation will last approximately 30 minutes and is intended to provide you with as clear and complete information as possible.

  • Scar revision is usually performed under local anesthesia. This means you can often go home half an hour after the procedure.

The plastic surgeon will make an incision in the marked area and remove the scar through this incision. It may not be sufficient to simply close the wound. There are certain techniques, such as the W- or Z-plasty, that the plastic surgeon can use to make the scar irregular. This technique will help it heal better.

After removing the old scar, the small wounds are sutured with a very thin, non-absorbable suture. A few small bandages are then applied over them.

  • You may experience increasing pain around the new scar shortly after the procedure. This is due to the anesthetic wearing off. Although this can feel uncomfortable, the pain is usually mild.

The treated area may still be slightly bruised, tender, and swollen. The same applies to the new scar, which may be red and raised for a few days. Don’t be alarmed by this; it’s a natural reaction of your body as it heals.

    Scar Cream

Your scars will take about one to one and a half years to fully heal. During this time, it’s important to always apply sunscreen with an SPF of 30 or higher. You should also keep your scars out of the sun and apply the scar cream thoroughly.

You’ll receive this nourishing cream to take home during your first follow-up visit. Once the wound has closed and the scabs have fallen off, you can start applying it. By applying it twice a day to the closed wound, you’ll increase the chances of a smooth, even healing.

  • It is important to realize that any surgical procedure can involve risks and complications. The risk is lower when correcting a scar because it is a minor procedure. In addition, we minimize potential risks to an absolute minimum through our professional approach.

    Possible complications may include:

    • Hematoma (a collection of blood under the skin that requires drainage)

    • Adverse reaction to the anesthesia

    • Postoperative bleeding

    • Infection

    • Permanent scarring

    • Unsatisfactory aesthetic result.

    Smoking

If you smoke or use tobacco or nicotine products at the time of your scar revision, you are at a higher risk of poor wound healing and skin necrosis. It can also have a negative effect on the anesthesia, potentially resulting in an increased risk of bleeding. We therefore strongly recommend that you stop smoking at least four weeks in advance and continue to abstain throughout your recovery process.

If you are exposed to smoke or tobacco products, you may also have an increased risk of complications compared to people who have no exposure. They have a significantly lower risk of such complications.

  • This can have various causes: certain parts of the body (such as the shoulder, back and breastbone) have an increased risk of scarring, as does a dark or, conversely, a very light, reddish skin colour. The direction of the scar in relation to the skin tension lines and the tension on the wound edges during recovery also play a role.
